How to Troubleshoot Incomplete Back to School Forms

Each year, the Back to School (BTS) Forms are opened prior to the start of school. Some forms are blank, and some have information from the previous year.
Each form is broken down into its own “step” and must be updated by parents with any necessary changes each year. Even if a form's fields are filled out, the system will not recognize it as “completed” until a Complete and Review button is clicked on for the corresponding step. This action works similarly as an electronic signature.
Guardians in Family 1 have to submit the entire form in order to get credit for completion. Many guardians mark all forms “complete” but never submit the packet in the last step.
This article provides essential information and helpful instructions for how to ensure the forms have been completed and the packet has been submitted properly. Please refer to the LISD website for even more Back-to-School resources.

Family Access
The following steps will be completed in Skyward Qmlativ SIS. You may need to click the
down arrow next to the
home icon and select Family Access from the drop-down menu.

Click the 20##/20## Back to School Forms tile.

The Back to School forms will be available to complete under each student name you have enrolled in LISD.

Please read the instructions on the main page and click Complete & Next to proceed through the numbered steps displayed in the left-hand menu.

Note
Depending on your student's grade level, you may have more or fewer steps. There are required steps that must be completed by law, and then there are optional steps that you choose to skip or fill out.

As you work through each step, remember to click the Complete & Next button at the top of each form to apply your changes.
- Required fields are indicated with a * red asterisk.
- Make sure you have verified your chosen options and scrolled to complete all required fields before clicking Complete & Next.

checkmark will display next to completed steps in the left-hand menu.

On the last step, click the the Complete & Review button at the top of each form.
- Scroll down to review all of the information and check the box to confirm everything is correct.

Then, click the Submit Form button at the top of the form.

Return to Dashboard beneath the message to return to the home screen.

Once submitted, forms will automatically be approved. The 20##/20## Back to School Forms tile will update to say “Approved” in the green status header along the top, and the date you submitted the forms will be displayed at the bottom of the tile.
If you see “In Progress” as the status of any form, please go back to that form to complete the submission process. You cannot submit your forms to the campus unless all required steps are completed.
How to Make Updates to Approved Forms
LISD guardians can make changes to submitted Back to School Forms in Family Access by clicking on the 20##/20## Back to School Forms tile with an Approved status along the top. Do NOT click the tile with an “Available” status.

Clicking on a form tile with an “Approved” status will automatically change the status of that form to “In Progress.” The following screenshots are for a high school student in the 2026-2027 school year; the number of steps you see on your forms may vary based on your student's grade level.
The following areas are the most common ones that guardians wish to update:

Publication of Student Information: enables guardians to determine whether their student's directory information can be shared for certain purposes
- Students will not be included in their school yearbook unless the District box is checked.
- Student Code of Conduct: requires guardians to read and acknowledge the quick guide of the student handbook, including the Authorized Use Policy (AUP)
- LISD Student Device Program: enables guardians to indicate whether or not they want the student to receive a district-issued device, and if so, whether or not they want to pay for insurance coverage
Select the step in the left-hand menu that you would like to make updates to.

You can expand the accordions below to learn more about the options available to select on the corresponding steps.
A. Publication of Student Information
The District box must be checked if you want your student included in the school yearbook.

B. Student Code of Conduct
All guardians must have “I agree” selected in the I have read the above Quick Guide and agree to the Student Code of Conduct drop-down.

C. LISD Student Device Program
Depending on whether or not you want your student to receive a district-issued device, and if so, whether or not you want to pay for insurance coverage.

After making all desired changes to the Back to School forms for this student, click the Complete & Review button at the top of the form.
Scroll down to review all of the information and check the box to confirm everything is correct. Then, click the Submit Form button at the top of the form.

You see a screen thanking you for submitting the form. You can then click
Return to Dashboard beneath the message to return to the home screen.
Once submitted, forms will automatically be approved. Please refresh the screen to ensure you are viewing the most up-to-date status of the tile you just submitted the form for. That 20##/20## Back to School Forms tile will update to say “Approved” in the green status header along the top, and the date you submitted the forms will be displayed at the bottom of the tile.
Administrative Access
The following information is only visible to LISD staff when logged into KHub.
Troubleshooting Tips
- Verify you are in the NEW Family Access site.

- Verify you are in the Family Access portal.

- If you have multiple students in the district, be sure to check below each student's name for the 20##/20## Back to School Forms tile. This will not display in the top-most groupings of tiles for "All Students."
Note
Only members of Family 1 have access to back-to-school forms for the associated students. Members of Family 2 cannot see or complete these forms in Family Access.
